How long does Levitra take to work?
Vardenafil reaches its highest blood level between 30 minutes and 2 hours after you swallow it, at a median of 60 minutes on an empty stomach. That is why the U.S. label says to take it about 60 minutes before sexual activity.1 Bayer's international leaflet allows as little as 25 minutes.2
Absorption is quick but incomplete. About 15% of an oral dose reaches the circulation intact. And the timing only matters if there is sexual stimulation, because without it the tablet does nothing.1
How long does a dose last?
Bayer gives Levitra a window of at least 4 to 5 hours after the dose.2 Within that time, stimulation can lead to an erection. The erection itself still fades after sex, the way it normally would.1
If an erection lasts more than 4 hours, get medical help right away. Untreated priapism can damage penile tissue and cause a permanent loss of potency.1

Vardenafil and its main breakdown product, M1, each have a terminal half-life of about 4 to 5 hours. The liver clears it, mostly through the enzyme CYP3A4. About 91% to 95% of a dose leaves in the stool as metabolites and only 2% to 6% in the urine.1
Some men keep it longer. In men 65 and older, total exposure was 52% higher than in men aged 18 to 45, and in moderate liver impairment it was 160% higher after a 10 mg dose. Ritonavir, an HIV drug, stretched the half-life to 26 hours, which is why men on ritonavir are limited to 2.5 mg in 72 hours.1
If you ever need emergency care for chest pain, tell the staff when you took your last tablet. The label has not worked out a safe interval before nitrates can be given.1
Does 20 mg last longer than 10 mg?
Not by the label's numbers. Vardenafil's pharmacokinetics are roughly dose proportional across 5 to 20 mg, which means a higher dose raises the blood level, while the 4 to 5 hour half-life is the figure given for the drug as a whole. Bayer's 25-minute to 4-5-hour window is written for Levitra at any strength.12
The side-effect rate does climb with dose. In the placebo-controlled trials, headache and flushing were among the effects that grew more frequent from 5 to 20 mg.1
Taking Levitra with food
You can take vardenafil with or without food. In two food-effect studies, a high-fat meal lowered its peak blood level by 18% to 50%, so a heavy, fatty dinner just before the dose may blunt it.1
In the four main efficacy trials, men took vardenafil without regard to meals, and every dose still beat placebo.1
Levitra and alcohol
In a label study of healthy volunteers, vardenafil 20 mg taken with alcohol did not deepen alcohol's blood-pressure drop over 4 hours. The amount was 0.5 g per kg of body weight, about 40 mL of pure alcohol for a 70 kg man. Neither drug changed the blood level of the other.1
That study measured blood pressure in healthy people at one amount of alcohol. It tells you nothing about heavier drinking, or about what alcohol does to erections.
Can I take Levitra every day?
One tablet a day is the most you can take, with doses at least 24 hours apart. The U.S. label writes vardenafil as an as-needed drug taken before sex and gives no daily schedule.1
Longer use has been studied. In the clinical program, over 2,200 men took vardenafil for 6 months or more and 880 for at least a year. In a study that continued dosing for 31 days, the blood-pressure response on day 31 matched day 1.1
How to take Levitra and make it more effective
Take one tablet about an hour before sex, with or without food, and expect it to need sexual stimulation. If 10 mg isn't giving you a result you're satisfied with, the label's route is a dose change through your prescriber, up to 20 mg. A second tablet the same day is not allowed. A few things work against you. A high-fat meal close to the dose lowers the peak. Grapefruit juice was not tested, but the label expects it to raise vardenafil levels like other CYP3A4 inhibitors, and side effects such as headache and flushing increased with dose in the trials.1

Most of it. With a half-life of about 4 to 5 hours, the blood level halves every 4 to 5 hours, so after 12 to 15 hours roughly an eighth of the peak is left. The label still counts the next tablet as a new dose, at least 24 hours after the last one.

The label can't say. Its alcohol study looked at blood pressure in healthy volunteers, not at erections, and found no added drop with about 40 mL of pure alcohol. What it does say is that stimulation is needed and the dose can be adjusted by your prescriber if results aren't satisfactory.
